Peaceful Living is in search of a Residential Program Coordinator to join our dedicated team. The primary responsibility of this role is to ensure the welfare and well-being of the residents we serve, aligning with the core philosophy, mission, and vision of Peaceful Living. The coordinator plays a pivotal role in the effective delivery of services within our 'Intermediate Care Facilities for Individuals with Intellectual Disabilities' (ICF/IID) program.
Key Responsibilities
- Conduct and finalize assessments for each resident, adhering to Department of Health regulations.
- Develop Individualized Program Plans (IPPs) for residents and facilitate team meetings as necessary, including scheduling 90-day IPP reviews.
- Ensure accurate completion of program documentation, incorporating data that reflects the criteria established in the IPP.
- Regularly observe residents across various program areas to evaluate the effectiveness of program implementation and staff training, making timely adjustments as needed.
- Collaborate with staff to assess the relevance and effectiveness of program plans in meeting the identified needs of residents.
- Identify the need for program modifications based on residents' performance outcomes.
- Address inconsistencies in staff training approaches or program implementation, facilitating necessary corrections.
- Oversee follow-up for all recommended services, equipment, and programs to ensure timely provision of necessary resources for residents.
- Coordinate both internal and external program services to support skill acquisition.
- Encourage competent interactions among Direct Support Professionals in program implementation and behavioral support management.
- Work alongside professional and non-professional staff to deliver direct and indirect services tailored to the needs of residents.
- Participate in the Human Rights Committee, providing documentation and recommendations as appropriate.
Qualifications
- A master's degree or higher from an accredited institution with at least 1 year of experience working directly with individuals with intellectual disabilities; or a bachelor's degree with 2 years of relevant experience; or an associate's degree with 60 credit hours and 4 years of experience in the field.
