Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Absolute Home Health. As a key member of our healthcare team, you will provide direct patient care under the supervision of a registered nurse and work collaboratively with other healthcare professionals to achieve positive patient outcomes.

Responsibilities
  • Provide skilled nursing care to clients as directed by the registered nurse and physician's plan of care.
  • Follow the plan of care, provide treatments, and work collaboratively with the healthcare team to help meet positive patient care outcomes.
  • Assist in planning and exchanging information with other healthcare professionals to achieve and maintain continuity of client care.
  • Prepare clinical progress notes and follow client care plans as developed by the registered nurse and according to agency policy.
  • Adhere to policies and procedures, and maintain a high level of professionalism and integrity.
Requirements
  • At least one year of Home Health experience, with Home Care Home Base experience a plus.
  • Excellent written and oral communication skills, with the ability to work well in a team environment and independently.
  • Great customer service skills, with a focus on providing high-quality patient care.
  • Computerized medical records experience, with a strong attention to detail and ability to prioritize workload.
  • OASIS knowledge and competency, with a strong understanding of healthcare policies and procedures.
