UPMC Home Health Care is seeking a dedicated full-time Physical Therapist Assistant to serve the Mercer and Crawford Counties.

This role operates during daylight hours, Monday through Friday, from 8:00 AM to 4:30 PM. The primary responsibility is to deliver Physical Therapy services to Agency patients and clients, adhering to the regulations set forth by Pennsylvania state licensing laws.

Key Responsibilities:

  • Participate in public health initiatives by identifying unmet needs within families and communities.
  • Make referrals to both internal and external providers to enhance the health and welfare of families and communities under the guidance of a Licensed Physical Therapist.
  • Support the Agency's Mission, Principles, Values, Ethics, and Standards of Behavior.
  • Conduct ongoing patient assessments and assist the Licensed Physical Therapist in recognizing patient needs through continuous observation.
  • Utilize appropriate teaching methods tailored to the patient's age, culture, language, and educational level.
  • Implement the physical therapy plan with objectives collaboratively established with patients and their families.
  • Maintain open lines of communication with the Licensed Physical Therapist regarding patient conditions and progress.
  • Identify and communicate the need for referrals to the Licensed Physical Therapist.
  • Engage in performance improvement activities and report safety issues as they arise.
  • Comply with Agency standards, including productivity and clinical competencies, and adhere to infection control principles.
  • Maintain professional relationships with customers, prioritizing patient satisfaction and addressing complaints as directed.
  • Enhance knowledge and skills through educational resources and participate in clinical conferences as needed.
  • Document patient care and outcomes accurately and within the established timeframes.
  • Promote safety and compliance with Agency services, including adherence to the patient Bill of Rights and confidentiality standards.

Qualifications:

  • Graduation from a physical therapist assistant program accredited by the Commission on Accreditation in Physical Therapy Education or an equivalent program.
  • Licensure or regulation in the state of practice.
  • A minimum of two years of relevant experience as a Physical Therapist Assistant, with verified patient-related acts.
  • Proficiency in computer skills within a Windows-based environment is preferred.
  • Possession of a valid driver's license and access to transportation.

Certifications:

  • Current certification in CPR or Basic Life Support (BLS).
  • Compliance with all necessary clearances and background checks.
