We are seeking a highly skilled and compassionate Medical Director to lead our PACE (Program of All-Inclusive Care for the Elderly) program at WelbeHealth. As a key member of our interdisciplinary team, you will be responsible for providing high-quality medical care to our seniors and overseeing the medical service team to promote quality and outcome goals.
Key Responsibilities- Medical Leadership: Assume accountability and responsibility for the medical care of WelbeHealth participants, overseeing the medical service team to promote quality and outcome goals.
- Clinic Operations: Monitor and lead WelbeHealth clinic operations to assure integration of medical services with other participant care services for participants.
- Team Management: Recruit, train and supervise PACE Physicians, Nurse Practitioners, Community-Based PACE Physicians and other contracted medical staff.
- Comprehensive Assessments: Perform comprehensive assessments for participants and coordinate with the IDT to develop a comprehensive care plan for each participant.
- Care Coordination: Be an engaged member of the IDT, communicating participant changes, collaborating on care planning arrangements, and coordinating 24-hour care delivery.
- Network Collaboration: Work collaboratively with network providers and other members of the IDT to ensure effective care coordination, particularly around hospital discharge, short-term, and long-term placement.
- Medical Specialist Oversight: Oversee participants' use of medical specialists and inpatient care.
- Rotating Call: Participate in rotating night and weekend call.
- Quality Improvement: Review outcome and utilization data, including adverse events, hospitalizations, and use of specialists, in collaboration with the IDT to identify opportunities for improved care and operations.
- Medical Degree: Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Board Certification: Board Certification – Internal Medicine or Family Medicine or Emergency Medicine Geriatrics (Preferred).
- State License: Active New York Physician License without restrictions.
- Certifications: State CDS Certification and DEA Registration.
- Transportation: Reliable means of transportation.
- Skills: Excellent clinical, organizational, and communication skills in settings with frail, elderly participants, their families, and the IDT.
- Knowledge: Current knowledge of chronic care/geriatric issues and best practices.
- Medical Insurance: Medical insurance coverage (Medical, Dental, Vision).
