Your role will include:
- Embodying the mission, vision, and values of Apree Health
- Delivering evidence-based primary care to 10 to 15 patients daily, emphasizing preventive care, managing chronic and acute conditions, and providing disease management services
- Collaborating with patients and your care team, including an onsite Whole Health Coach, to create effective care plans tailored to each patient's unique needs
- Ensuring comprehensive care aligned with the Apree health model, which includes gathering health histories, performing physical assessments, conducting tests, and prescribing medications – all facilitated by onsite laboratory and pharmacy services
- Evaluating patient population needs and collaborating with team members to encourage participation in health assessments, coaching, and other wellness initiatives
- Providing clinical leadership through active participation in team meetings, proactive communication, prioritizing key initiatives, process enhancement, and supporting organizational change
- Ensuring that all patient expectations are met in collaboration with your care team
The ideal candidate will possess:
- MD or DO degree
- Completion of a Family Medicine or Internal Medicine Residency
- Current Board Certification by ABMS or AOA
- A minimum of 3 years of experience in primary care
- A valid and unrestricted medical license and DEA number
- Experience in team-based care
- Familiarity with EMR systems
- Knowledge of coding and documentation, including Medicare Advantage and HCC
- Proficiency with Google Workspace
Compensation: $238k-$350k annual salary
