Primary Care Physician, Family or Internal Medicine

At Apree Health, our providers are dedicated to fostering meaningful connections with patients, addressing their comprehensive health needs rather than merely treating symptoms. You will engage with 10 to 15 patients daily, conducting appointments that last between 30 to 60 minutes, all while being supported by a collaborative care team, including health coaches, to deliver holistic healthcare – a service often relegated to specialists in the current healthcare landscape. Key Responsibilities

Your role will include:

  • Embodying the mission, vision, and values of Apree Health
  • Delivering evidence-based primary care to 10 to 15 patients daily, emphasizing preventive care, managing chronic and acute conditions, and providing disease management services
  • Collaborating with patients and your care team, including an onsite Whole Health Coach, to create effective care plans tailored to each patient's unique needs
  • Ensuring comprehensive care aligned with the Apree health model, which includes gathering health histories, performing physical assessments, conducting tests, and prescribing medications – all facilitated by onsite laboratory and pharmacy services
  • Evaluating patient population needs and collaborating with team members to encourage participation in health assessments, coaching, and other wellness initiatives
  • Providing clinical leadership through active participation in team meetings, proactive communication, prioritizing key initiatives, process enhancement, and supporting organizational change
  • Ensuring that all patient expectations are met in collaboration with your care team
Qualifications

The ideal candidate will possess:

  • MD or DO degree
  • Completion of a Family Medicine or Internal Medicine Residency
  • Current Board Certification by ABMS or AOA
  • A minimum of 3 years of experience in primary care
  • A valid and unrestricted medical license and DEA number
  • Experience in team-based care
  • Familiarity with EMR systems
  • Knowledge of coding and documentation, including Medicare Advantage and HCC
  • Proficiency with Google Workspace

Compensation: $238k-$350k annual salary
