Job Summary

Pathways Home Health, Hospice and Private Duty is seeking a skilled Nurse Case Manager to join our team. As a Registered Nurse, you will be responsible for providing high-quality patient care and coordinating services with our multidisciplinary team.

Key Responsibilities
  • Conduct comprehensive assessments of patients and their families to identify physical, psychosocial, and environmental needs.
  • Develop and implement individualized plans of care based on assessments and document observations and interventions.
  • Coordinate care with the clinical team, including physicians, therapists, and home health aides.
  • Provide skilled care and procedures, including teaching patients and families.
  • Obtain and document physician orders and perform home health aide supervisory visits.
  • Collaborate with the clinical team manager to address scheduling needs and submit weekly visit schedules.
  • Participate in quality improvement activities and assist in the development of agency protocols and policies.
Requirements
  • Current California RN license
  • Minimum of one year of nursing experience
  • Current CPR Certificate
  • Current California driver's license and automobile insurance
