Key Responsibilities:- Develop and implement individualized care plans to address the social and emotional needs of residents.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support.
- Work with residents to identify and address social and emotional needs, and develop strategies to promote independence and well-being.
- Participate in care plan meetings and provide input on resident care and support.
- Work with the finance team to ensure residents receive necessary resources and support.
- Coordinate discharge planning and ensure residents receive necessary services and support upon discharge.
- BSW or MSW preferred, or Bachelor's degree in a related field.
- At least 3 years of experience in social work or a related field.
- Ability to work flexible schedule.
- PCC experience preferred.
