The successful candidate will be responsible for evaluating patients' conditions, developing treatment plans, and instructing nursing staff and patient families on follow-through programs. Key responsibilities include:
- Evaluating patients' conditions and developing tailored treatment plans to promote recovery and independence
- Instructing nursing staff and patient families on follow-through programs to reinforce progress and achieve optimal outcomes
- Documenting patient care in accordance with regulatory, licensing, payer, and accrediting requirements
- Maintaining equipment and work areas in a safe and clean condition
