- Conduct thorough evaluations of patients' physical function, mobility, and range of motion, identifying areas for improvement and developing targeted treatment plans.
- Direct and implement physical therapy interventions, including exercises, modalities, and education, to promote patient recovery and independence.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
- Document patient progress, outcomes, and treatment plans, maintaining accurate and comprehensive records.

- Supervise and mentor physical therapy assistants and students, promoting professional growth and development.
- Current licensure as a Physical Therapist in the state of practice.
- Graduate degree in Physical Therapy from an accredited program.
- Minimum of 1 year of experience in home health or hospice care, with a focus on physical therapy.
