As a Home Health Physical Therapy Assistant, you will work closely with our Physical Therapists to develop and implement personalized treatment plans that promote patient independence and well-being.
Responsibilities- Provide evidence-based physical therapy interventions to patients in their homes, following established treatment plans and protocols.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
- Collaborate with Physical Therapists, nurses, and other healthcare professionals to ensure seamless care coordination and communication.
- Document patient progress and outcomes accurately and thoroughly, using standardized assessment tools and reporting procedures.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Current licensure, registration, or certification to practice as a Physical Therapy Assistant in the state of Missouri.
- Current CPR certification and ability to obtain and maintain a valid driver's license.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to lift, push, and pull equipment and supplies as needed, with or without reasonable accommodation.
