We are seeking a skilled and compassionate Hospice Nurse Admissions Specialist to join our team at Amedisys, Inc.
Key Responsibilities- Coordinate admissions of hospice patients by conducting thorough assessments of their physical, psychosocial, and spiritual needs.
- Collaborate with the attending physician, medical director, and interdisciplinary team to establish individualized plans of care.
- Provide skilled interventions and instructions to patients and caregivers to ensure optimal symptom control and patient comfort under the direction of the physician.
- Serve as a liaison between disciplines and facilities during the admission process.
- Develop, prepare, and maintain accurate and timely patient care progress records.
- Communicate significant findings, problems, changes in condition, or environment to the clinical manager/physician and other members of the hospice care team.
- Perform on-call responsibilities and provide on-call services to patients/families as assigned.
- Current RN license, specific to the state(s) assigned to work.
- One year of clinical RN experience.
- Current CPR certification.
