The Orthopedic Nurse Navigator RN is a key member of our healthcare team, responsible for coordinating patient care throughout the continuum of health. This role requires a strong nursing background, excellent communication skills, and the ability to work independently with minimal supervision.
Key Responsibilities- Collaborate with the orthopedic team to provide seamless care to patients during their episode of care.
- Educate and collaborate with orthopedic staff, physicians, and the community on services and resources available.
- Implement chronic care management plans to ensure optimal quality of life outcomes.
- Assist patients and their caregivers in understanding their diagnosis, treatment options, and resources available.
- Assist with the education of staff members on best practice standards and order sets in consensus with national guidelines.
- Function as an expert clinician and mentor for staff.
- Assist in tracking data, outcomes, and supporting strategic and performance improvement activities.
- Practice LEAN principles to streamline care across the continuum, being mindful of cost and quality outcomes.
- Lead education groups and monitor outcomes, such as patient satisfaction and compliance with treatment regimens.
- Current licensure as a Registered Nurse in the state of Georgia.
- Orthopedic Certification (ONC) within six months of hire.
- Belong to a National Association of Orthopedic Nurses (NAON) to stay abreast of current evidence-based practice.
- Bachelor's Degree in Nursing.
- Minimum of two years of direct patient care experience in orthopedic nursing, including providing education to patients and families.
