We are seeking a dedicated and compassionate Registered Nurse to fill the role of Weekend On-Call Hospice Nursing Professional at Hospice of Wilson Medical Center.

About the Position

This salaried position involves working an on-call schedule from Friday 4:30PM to Monday 8AM, with potential visits required during the day on Saturday and Sunday. The ideal candidate will have a strong foundation in hospice care and be comfortable working independently.

Responsibilities will include delivering high-quality hospice care to patients and their families, triaging patient needs over the phone, and responding to crisis calls with urgency. You will also conduct initial nursing evaluations, perform afterhours visits, and document patient progress and interventions.

Key Responsibilities
  • Provide comprehensive and timely hospice care to patients and their families.
  • Triage patient needs over the phone and respond to crisis calls with urgency.
  • Conduct initial nursing evaluations to determine eligibility for hospice services.
  • Perform afterhours visits, including admission visits and death visits.
  • Assess and reassess patient/family needs as needed.
  • Document patient/family progress, problems, goals, interventions, and response to hospice care.
Requirements
  • A minimum of one year of clinical experience.
  • Current RN licensure in the state of practice.
  • Current CPR Certification.
  • A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
