Job Title: Licensed Practical Nurses/Licensed Vocational Nurses

About the Role:

We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team as a Health and Wellness Director. This is a temporary position with a 7-month contract.

Key Responsibilities:
  • Foster a resident-centered approach to care, ensuring residents' rights and dignity are respected.
  • Assess residents' health, functional, and psychosocial status, developing individualized service plans and managing care services.
  • Establish and maintain the medication program, ensuring compliance with regulations.
  • Oversee hiring, promotions, and disciplinary actions for licensed nursing staff and care staff.
  • Provide hands-on training, supervision, and staff development.
  • Communicate with residents' families regarding healthcare needs.
  • Maintain current departmental policies, procedures, and licenses.
  • Ensure accurate and up-to-date resident records and charting.
  • Participate in marketing efforts, representing the community positively to outside agencies.
Requirements:
  • Positive team player mentality and passion for serving seniors.
  • LVN/LPN or RN nursing license in good standing.
  • Minimum 4 years of geriatric nursing experience, including medication administration and delivery systems.
  • Minimum 2 years of supervisory experience, with skills in budgeting, staff development, training, and scheduling.
  • Associate degree or higher required; additional coursework in management is an asset.
  • Experience in Memory Care is preferred.
  • Outstanding organizational skills and proficiency in Microsoft suite.
  • Maintains current state-required certifications, such as First Aid and CPR.
  • Ability to work flexible hours, including weekends, holidays, and overtime.
  • Capacity to be on-call 24/7.
  • Meets all mandatory health requirements by state regulations.
  • Compassion and interest in working with seniors and individuals with memory loss.
