Diabetes Educator

Job Purpose:
As a Diabetes Educator at Piedmont Macon Medical Center, you will play a vital role in providing direct patient care in acute care and ambulatory settings.
Key Responsibilities:
* Provide direct patient care in acute care and ambulatory settings based on consultative requests from the medical staff.
* Work closely with the entire medical staff and nursing staff to prepare patients for discharge in a safe and efficient manner.
* Provide tools and skills for homecare to patients and families.
* Make recommendations to hospital staff for changes in diabetes management.
* Serve as an educational and informational resource to all hospital staff and the community.
Requirements:
* Graduate of a Nursing program or Bachelor's of Science in an applicable field.
* Current License as a Registered Nurse (RN), Registered Dietitian (RD), Certified Diabetes Educator (CDE), or Diabetes Care and Education Specialist (CDCES) required.
* Two (2) years of experience in a clinical setting preferred.
* Experience in Diabetes Education preferred.
