Job Description:

To succeed in this role, candidates must ensure that all necessary documents are attached upon submission, including a completed prescreen and verification of state license. New graduates are welcome to apply, with SNF experience preferred. Prior to the start date, a Covid test with results issued 48 hours prior to the start date is required. Before submitting, please review the credentialing checklist, which includes state license verification, resume, references, county and federal criminal searches, and other necessary documents. This position requires a strong understanding of dementia training and the ability to work normal shifts, Monday through Friday. The ideal candidate will have a minimum of 1 year of experience and be currently active in their state license. This is a rehabilitation therapy sub-specialty position with no EMR used.
