Job Summary:
Meridian Health Services is seeking a skilled Skill Builder to join our team. As a Skill Builder, you will provide skills and development training to assist patients and/or families in developing skills directed towards achieving developmental milestones and reducing psychosocial barriers.
Key Responsibilities:
- Assist the Therapist in developing, implementing and revising individual treatment plans.
- Provide individual and group skills training and development to assist the patient and/or family in developing skills directed toward reducing psychosocial barriers.
- Provide case management services as identified in the patient's treatment plan.
- Work collaboratively with the Therapist and medical staff to ensure coordination of integrated care for the patient.
- Complete CANS/ANSA re-assessments.
- Provide consultation to staff on specific cases involving coordination of care.
- Maintain positive working relationships with staff of other agencies and institutions, homes, and facilities, and act as a liaison between patients and agencies and/or institutions.
- Prepare and maintain clinical records, reports and forms and other administrative tasks as required.
Requirements:
- High school diploma or its equivalent with 2 years paid behavioral healthcare experience or equivalent college coursework toward a human services or health related degree required at minimum.
- Associate or Bachelor's degree preferred.
- A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
- A commitment to providing individual attention to help clients achieve mental, emotional and physical well-being.
