Requirements
  • Education and Licensure
    • Active Nurse Practitioner license in the state of Michigan
    • AANC or AANP board certification for Nurse Practitioners
  • Experience
    • 2+ years of clinical experience in providing primary care in an internal medicine, family medicine, or geriatric medicine setting
    • Proven skills, knowledge base, and judgment necessary for independent clinical assessment and decision-making of acute and non-acute clinical problems
  • Skills and Abilities
    • Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations and healthcare teams
    • Strong analytical and problem-solving skills, with the ability to interpret complex patient data and develop effective care plans
Preferred Qualifications
  • Experience in Adult Internal Medicine, Family Medicine, Geriatrics, Addiction Medicine, and Palliative Care
    • Experience providing home-based care and working with individuals with complex chronic medical, behavioral, and social needs
    • Experience engaging individuals with untreated and/or symptomatic chronic mental illness and addiction
  • Value-Based Care Experience
    • Experience working in value-based care organizations and contributing to quality improvement initiatives
