Nurse Practitioner

We are seeking a skilled Registered Nurse to join our Home Health team. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes. This is a great opportunity for a nurse who wants to make a difference in the lives of our patients and their families.

Responsibilities

• Conduct comprehensive assessments of patients to develop individualized care plans.
• Provide skilled nursing care to patients in their homes, as prescribed by physicians.
• Collaborate with other healthcare professionals to ensure continuity of care.
• Document patient progress and communicate with physicians and other healthcare professionals as needed.

Requirements

• Current RN licensure in the state of practice.
• Current CPR certification.
• At least one year of clinical experience as a Registered Nurse.
