We are seeking a compassionate and skilled Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing personalized care to patients in their homes, helping them maintain their independence and quality of life.
Key Responsibilities- Provide direct patient care under the supervision of a registered nurse and according to the patient's care plan.
- Assist patients with daily living activities, such as bathing, grooming, and dressing.
- Help patients maintain good personal hygiene and provide basic care, such as shaving, oral care, and nail care.
- Assist patients with mobility, exercises, and transfers, as per the patient's care plan.
- Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care.
- Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
- At least one year of experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
- Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare-certified agency.
