As a Physical Therapist Case Manager, you will play a vital role in our team, working closely with patients, physicians, and other healthcare professionals to provide high-quality care.

Key Responsibilities:
  • Evaluate patients in their homes and develop personalized care plans to address their unique needs and goals.
  • Provide skilled physical therapy services to help patients regain their independence and improve their quality of life.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Supervise Physical Therapy Assistants (PTAs) and provide guidance and support as needed.
  • Document patient progress and outcomes, and participate in quality improvement initiatives.

Requirements:

  • Current unrestricted California Physical Therapist License
  • Current BLS/CPR Certification
  • Valid California Driver's License
