In this role, you will deliver compassionate care to assigned residents, ensuring their safety and well-being while adhering to established standards and individualized care plans.
Conduct thorough nursing assessments for all residents upon admission and continuously, in compliance with state and federal regulations, utilizing the nursing process which includes assessment, diagnosis, intervention, and evaluation.
Exhibit proficiency in clinical skills as outlined in the Competency Verification and Training Checklist. Ensure that residents receive prescribed medications, treatments, and procedures promptly, following appropriate infection control protocols. Gather necessary equipment and supplies to facilitate care as required. Accurately perform glucometer testing and control monitoring as per established procedures. Implement tailored strategies to manage behaviors and enhance residents' emotional well-being according to their care plans. Ensure comprehensive documentation of resident care, updating records as necessary, and reporting any changes in condition according to established policies.Record incidents or accidents in resident files and complete all required documentation (incident reports, etc.). Participate in quality improvement initiatives, including audits and corrective actions as assigned.
May be responsible for completing designated sections of the MDS for Nursing and signing off on MDS forms to confirm the process is completed.
Ensure timely processing of physician orders and notify physicians as needed. Attend or obtain resident reports prior to starting your shift, monitor and report on residents' conditions, and respond promptly to resident call lights. Provide education to residents and their families/significant others. Ensure timely follow-up on signs and symptoms of resident infections.Must be a graduate of an accredited nursing program. Previous experience in a similar role is preferred.
