Job Summary:As a Community Care Worker I with the Texas Department of Aging & Disability Services, you will play a vital role in determining the eligibility of individuals for community care services. This position requires a highly motivated and compassionate individual who can effectively communicate with clients, assess their needs, and develop personalized service plans.
Key Responsibilities:
•Conduct home visits and interviews to assess the needs of individuals and determine eligibility for community care services.
•Develop and implement service plans that meet the unique needs of each individual, ensuring they receive the necessary support to maintain their independence and quality of life.
•Collaborate with clients, family members, and healthcare providers to ensure a comprehensive understanding of each individual's needs and goals.
•Maintain accurate and up-to-date records of client interactions, service plans, and outcomes, using automated systems to track progress and identify areas for improvement.
•Communicate effectively with clients, colleagues, and other stakeholders to ensure seamless service delivery and coordination.
•Stay up-to-date with policy regulations, program requirements, and best practices in community care, applying this knowledge to inform service delivery and decision-making.

Requirements:
•High school diploma or equivalent required; degree in social work, gerontology, or related field preferred.
