Job Description:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as an LPN Mental Health Outpatient Coordinator. This is an exciting opportunity for a dedicated professional to provide high-quality patient care and support in a dynamic mental health outpatient setting.

Responsibilities:

  • Provide direct patient care to individuals with mental health conditions, including medication management, wound care, and vital sign monitoring
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet those needs
  • Collaborate with interdisciplinary teams, including psychologists, psychiatrists, and social workers, to ensure comprehensive and coordinated care
  • Document patient interactions, progress, and outcomes accurately and thoroughly, maintaining accurate and up-to-date medical records
  • Participate in ongoing education and training to enhance skills and knowledge in areas such as crisis intervention, cultural competence, and evidence-based treatments

Requirements:

  • Degree from an accredited LPN program
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of employment
  • Minimum 2 years of experience in a mental health or healthcare setting
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations
  • Ability to lift, push, and pull objects weighing up to 50 pounds, stand for prolonged periods, and perform other physically demanding tasks as required

Preferred Qualifications:

  • Bachelor's degree in Nursing or related field
  • Experience working with geriatric, adolescent, or adult populations
  • Knowledge of crisis intervention techniques and trauma-informed care

Working Conditions:

  • This position requires regular attendance and punctuality, as well as flexibility to adapt to changing situations and priorities
  • May require rotating shifts, weekends, and holidays

Salary: $64,625 - $83,615 per annum
