Job Description

In this role, you will be responsible for providing high-quality patient care and supporting patients in their recovery journey. You will work closely with the care team to develop and implement individualized treatment plans and ensure seamless care coordination.

Responsibilities

  • Administer physical therapy treatments as developed by the Physical Therapist.
  • Document patient progress and maintain accurate records.
  • Communicate effectively with patients, families, and healthcare providers.
  • Participate in ongoing education and training opportunities.

Requirements

  • Bachelor's degree in Physical Therapy from an accredited institution.
  • State licensure to practice as a Physical Therapist Assistant.
  • Two years of experience in a related field, preferably in home health care.
  • Certification in CPR and first aid.
