Job SummaryWe're seeking a part-time Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to join our expanding house call practice in the Philadelphia/Center City area. As a key member of our team, you'll provide primary care services to patients in their homes, Board and Care facilities, and Assisted Living facilities.
Responsibilities- Provide primary care services to patients in their homes, including health education and management of chronic health problems.
- Drive to patient locations in your assigned geographic area, with an average of 7-10 patient visits per day.
- Exhibit exceptional bedside manner and communicate complex medical information in a clear and compassionate manner.
- Work with patients to prevent illness, promote health, and manage chronic health problems.
- Provide referrals to appropriate medical resources and utilize our EMR system to document patient visits accurately and timely.
- Participate in an on-call rotation and perform other duties as assigned.
- Graduate of an accredited Physician Assistant program or Nurse Practitioner program.
- Current, unrestricted PA or NP license to practice in Pennsylvania, including DEA license.
- Minimum of 1 year of experience in primary care, hospice, or urgent care/outpatient setting.
- Previous experience in a home health, hospice, or other home visit setting is highly desired.
- Enrolled in Medicare preferred, Keystone credentialed preferred, and PA Medicaid enrollment preferred.
- Competitive hourly rate: $65.00 - $85.00 per hour.
