Job DescriptionThis is a challenging yet rewarding role that requires a registered nurse to provide direct patient care in the patient's home. The ideal candidate will have excellent communication and interpersonal skills, a strong understanding of patient-centered care, and the ability to work effectively in a fast-paced environment.
Responsibilities- Develop and implement individualized care plans for patients.
- Conduct thorough assessments and evaluations of patients' physical and emotional needs.
- Provide education and training to patients and their families on disease management and self-care techniques.
- Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care.
- Maintain accurate and detailed records of patient interactions and care delivered.
- Stay up-to-date with relevant laws, regulations, and industry developments.
- Minimum of Associate degree (or higher) in Nursing.
- Current and unrestricted California Registered Nurse license.
- Current CPR/BLS certification.
- Minimum of 1 year RN experience in an institutional setting. Acute Care/Rehab/Hospital/Home Health/Hospice.
- Excellent communication and interpersonal skills.
- Strong analytical and problem-solving skills.
Competitive salary range of $80,000 - $110,000 per year, plus benefits package including paid time off, holidays, sick leave, medical insurance, dental insurance, vision insurance, life insurance, and 401(k). Opportunity for career growth and professional development.
