Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct comprehensive health assessments, develop care plans, and provide education to patients and their families. You will work collaboratively with our team of clinicians and healthcare professionals to deliver exceptional care and improve health outcomes.

• Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
• Develop and implement care plans to improve patient outcomes
• Provide education to patients and their families on health management and disease prevention
• Collaborate with primary care physicians and other healthcare professionals to ensure seamless care coordination
• Work in a variety of settings, including home visits, skilled facilities, and telehealth

• Masters degree in nursing or related field
• Current RN and NP licensure in state of practice
• Board certification in a Matrix-approved specialty
• Minimum of 1 year experience as a Nurse Practitioner
• Strong computer skills and familiarity with electronic health records
