Registered Nurse Case Manager

As a Home Health Care Case Manager, you will be responsible for delivering skilled nursing assessments and services to patients in their residences across various counties.
Key Responsibilities:
- Conduct thorough nursing assessments and document care provided.
- Communicate effectively with the patient's physician regarding care and treatment plans.
- Coordinate interdisciplinary management, including collaboration with Physical and Speech Therapy.
- Maintain flexibility in scheduling visits and adjust daily plans as necessary.
Essential Skills:
- Proficient in nursing assessment and concise communication.
- Experience with IV therapy, including Mid, PICC, and Central line management.
- Competence in medication management and wound care, including wound vac care.
- Ability to provide verbal patient assessments to physicians and implement orders.
- Familiarity with computerized assessment tools and ability to work independently with excellent time management.
Requirements:
- Valid Ohio Registered Nurse license.
- Valid Ohio driver's license and reliable transportation.
- Previous nursing experience is highly recommended.
- Wound care experience is essential.
- Ability to work collaboratively as part of a healthcare team.
- Knowledge of OASIS assessment protocols is a plus.
Work Schedule:
- Full-time position with a rotating evening, weekend, and holiday schedule.
Compensation and Benefits:
- Competitive salary of $75,000.00 per year.
- Benefits include health insurance, dental insurance, mileage reimbursement, and 401k.
- Paid time off and flexible scheduling options.
Work Environment:
- Position requires daily travel to patient homes, potentially up to 30 miles from your residence.
Certification Requirements:
- CPR certification and health screening are mandatory prior to employment.
- Local and FBI background checks are required.
