Job Summary

We are seeking a skilled and compassionate LPN to join our team as a Home Health Care Specialist. In this role, you will work closely with our interdisciplinary team to provide comprehensive care to patients in their homes, focusing on geriatric care and complex needs.

Key Responsibilities
  • Partner with our team to develop and implement individualized care plans for patients, ensuring their unique needs are met.
  • Provide clinical services and coordination in the home setting, including wound care, medication management, and patient status checks.
  • Conduct health screenings and assessments to identify potential health risks and develop strategies for prevention and intervention.
  • Collaborate with our interdisciplinary team to ensure seamless care transitions and optimal patient outcomes.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
  • Current LPN licensure in the state of employment.
  • Experience in geriatric care and home health settings.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to travel to patient homes and work in a fast-paced, dynamic environment.
