We are seeking a compassionate and skilled RN Case Manager to join our team at Assured Home Health & Hospice.

Salary Range: $55,000 - $60,000 per year (DOE)

In this rewarding role, you will assume full nursing responsibility for delivering high-quality care to hospice patients and their families. You will work closely with the patient, family, attending physician, and other members of the Interdisciplinary Group (IDG) to provide comprehensive care that meets each patient's unique needs.

  • Makes initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
  • Identifies patients' physical, psychosocial, and environmental needs, reassessing as needed, no less than every 15 days.
  • Initiates and coordinates plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition.
