Job Summary: We are seeking a highly skilled Patient Placement Specialist to join our team at Ochsner Health. As a key member of our healthcare operations team, you will play a critical role in coordinating patient placement and transportation across our network.

Key Responsibilities:

  • Coordinate transportation for all transport orders through acting as a dispatcher.
  • Respond to system-wide requests for acute behavioral health placement, ensuring all necessary patient referral information is complete, electronically submitting requests for evaluations of patient criteria to determine if admission criteria are met, organizing and prioritizing referrals to promote patient safety and meet patient needs within the continuum of care.
  • Provide excellent customer service through telephone and personal contact with all customer groups including physicians, nurses, administrators, and colleagues.
  • Accommodate fluctuating variables in practice setting such as patient volume, patient acuity, experience of personnel, utilization of supplies, and resources.
  • Communicate pertinent clinical information to referring providers, admitting providers, and administrative staff and assist with answering transfer center calls, creating tickets in Epic, and performing follow-up calls for reassessment of patients.
  • Facilitate time-sensitive tele-medicine consultations between campuses and providers, ensuring completion of all workflow items as part of the transfer center team.
  • Act as part of the centralized placement team through assigning beds for all campuses across the system from every entry point, including controlling the bed board for community hospitals to ensure every patient receives a bed in a timely manner.

Requirements:

  • Completion of an accredited licensed practical nurse (LPN) program or a bachelor's degree.
  • 2 years of licensed practical nurse (LPN) experience. Bachelor's degree accepted in lieu of experience.
  • Current licensed practical nurse (LPN) license in the state of practice if accredited LPN program has been completed.
  • Proficiency in using computers, software, and web-based applications.
  • Effective verbal and written communication skills and ability to present information clearly and professionally.
  • Excellent customer service phone skills and ability to multi-task.
  • Ability to be self-directed and motivated and work with little supervision.
  • Ability to demonstrate effective adaptation skills to change in systems, practices, and technology.
  • Ability to work extended shifts of 8, 10, or 12 hours per day.
