Career Opportunity:
PASCO is currently in search of a compassionate and dedicated Registered Nurse (RN) to provide personalized, one-on-one care for a young client. This role involves delivering specialized care, including respiratory and GJ care.

Position Details:
This position offers flexibility, allowing for part-time or full-time work with day shifts typically ranging from 10 to 12 hours. Shifts may include hours such as 8 AM to 6 PM.

Key Responsibilities:
- Deliver direct and comprehensive nursing services based on the client's physician's orders and established standards of practice.
- Conduct thorough assessments that encompass physical, emotional, psychosocial, and environmental aspects of the client's needs, initiating appropriate preventive and rehabilitative nursing procedures.
- Ensure high-quality care by effectively communicating and coordinating with the RN Case Manager, family members, physicians, and other nursing staff.
- Maintain accurate in-home charts and medication records, promptly updating the RN Case Manager on any changes.
- Demonstrate a thorough understanding of the client's individualized Plan of Care and recommend necessary adjustments when appropriate.
- Supervise and guide assigned LPNs and aides in accordance with regulatory standards and agency policies.
- Uphold care quality standards and complete CNA supervisory forms within designated timelines.
- Identify and address the learning needs of clients and caregivers, providing education and referrals to community resources as needed.
- Maintain open communication with team members and nursing administration, providing essential information regarding client health status.

Qualifications:
- Documented experience and skill set aligned with client and family needs.
- Home health nursing experience is highly preferred.

Essential Skills:
- Ability to independently gather and interpret client data, arriving at informed conclusions and actions.
- Knowledge of unique care delivery issues for individuals with various disabilities.
- Proficient communication skills, both verbal and written.
- Familiarity with computer operations, including Microsoft products and Electronic Health Record (EHR) systems.
- Ability to perform tasks requiring significant walking or standing during shifts.
