Patient Care Logistics Expert Job Summary

We are seeking an experienced and detail-oriented individual to join our team as a Patient Care Logistics Expert. In this role, you will assume accountability for patient plans of care, providing and managing care to individuals and cohorts of patients within a unit.

You will design, implement, and evaluate nursing interventions by coordinating, delegating, and supervising the care provided by the healthcare team. Your expertise will facilitate effective and efficient discharge planning, ensuring seamless transitions for our patients.

As a key member of our team, you will serve as a communication link between Unit Director, staff, patients, physicians, and other stakeholders. You will also provide a significant resource to staff for patient care assessment and planning, problem resolution, and quality improvement initiatives.

To succeed in this role, you should have a registered nurse diploma and at least 2 years of relevant experience working in a fast-paced, stressful environment. A current RN license in the state of practice and Basic Life Support certification through the American Heart Association are also required.

As a Patient Care Logistics Expert, you will have the opportunity to work collaboratively with other healthcare professionals to improve patient outcomes and enhance our organization's reputation for excellence.
