We are seeking a skilled Medical Case Manager to join our team at Insight Global.
Responsibilities:- Identify patients at high risk and collaborate with providers and clinic staff to develop effective care plans.
- Support chronic care patients by conducting thorough health assessments, reconciling medications, and setting self-management goals.
- Provide patient education and referrals to promote optimal health outcomes.
- Assist in transitioning patients from inpatient to outpatient care by assessing needs, reconciling medications, and arranging post-discharge support.
- Collaborate with providers to coordinate care plans and ensure seamless transitions.
- Maintain accurate documentation in the electronic medical record.
- Participate in daily huddles and Patient Care Coordination meetings.
- Prepare reports for weekly meetings to track progress and identify areas for improvement.
- Maintain up-to-date knowledge of evidence-based practices and perform clinical tasks within scope of practice.
- Licensed nurse with experience in case management.
- Bilingual in English/Spanish.
- At least 1 year of experience in case management.
**Estimated Salary: $55,000 - $65,000 per year**, depending on location and experience. We offer a competitive compensation package and opportunities for growth and development.
