BAYADA Home Health Care is seeking a skilled Registered Nurse (RN) to provide high-quality care to patients in their homes in Monroe County, Pennsylvania.

Key Responsibilities:

  • Provide comprehensive care to patients in their homes, including administration of medication, wound care, and treatments
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Document patient care and progress accurately and thoroughly

Requirements:

  • Graduation from an accredited and approved nursing program
  • Minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Prior home care experience strongly preferred, but not required
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred
