Key Responsibilities:
  • Provide individual, family, or group modalities to patients and families
  • Practice within the guidelines of professional social work ethics and standards
  • Participate in multi-discipline case conferences
  • Consult with agency care providers regarding social service needs of clients and families
  • Conduct psychosocial assessments and interventions in crisis situations
  • Develop and implement in-home care plans
  • Provide information, referral, and creative resource acquisition for specific individual and family needs
  • Collaborate with the healthcare team to promote a smooth, coordinated plan of care
Requirements:
  • Masters degree in Social Work from an accredited institution
  • Ability to work in a fast-paced environment
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
