About the Role:- Provide effective and equitable value-based primary care to adults on Medicare
- See patients independently and collaborate with physicians as needed
- Build meaningful patient relationships and provide longitudinal care
- Practice in our outpatient settings as part of a collaborative care team
- Provision of exceptional primary care
- Conduct office visits for routine and acute issues
- Administer Annual wellness visits and health risk assessments
- Care coordination with other providers and specialists
- Population health leadership and education
- Master of Science in Nursing or Doctor of Nursing Practice
- National certification as a Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- Experience in primary care: internal medicine, geriatrics, or family medicine
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments
- Passion for teamwork and collaboration
