About the Role

Amedisys, a leading home health and hospice company, is seeking a skilled Hospice Liaison to join their team. As a key member of the sales team, you will be responsible for developing and maintaining relationships with healthcare professionals, patients, and families to promote home care services.

Key Responsibilities
  • Educate healthcare professionals, patients, and families about home care services
  • Develop and maintain relationships with key referral sources, such as hospitals, nursing homes, and physician offices
  • Coordinate patient admissions with the team, including obtaining required documentation
  • Conduct on-site visits to assess patient needs and identify barriers to care
  • Keep accurate records of patient interactions and referrals
  • Achieve admissions, product, and payor-mix goals
  • Meet short and long-term target account goals
  • Document sales calls and maintain a complete record of targeted referral sources
  • Attend community events to promote Amedisys
  • Respond to customer complaints in a timely manner
Requirements
  • At least one year of experience in a healthcare-related business development/outreach role
  • Bachelor's degree preferred, but not required
  • Reliable transportation, current driver's license, and liability insurance
  • Ability to travel 50% or more within the assigned territory
