The CCT Transition Coordinator role at Libertana Home Health is a rewarding opportunity to make a meaningful difference in the lives of seniors. As part of our team, you will be responsible for coordinating housing solutions for residents transitioning from skilled nursing facilities.

Key Responsibilities:
  • Assist clients with housing and transition coordination
  • Establish working relationships with SNF case managers, discharge planners, and representatives to educate them about the CCT program and available resources
  • Identify and interview residents for pre-screening, isolating client needs and preferences

In this role, you will work closely with our agency RNs to assess residents, prepare initial care plans, and submit documentation to DHCS for review. You will also apply for appropriate waivers based on resident needs, work on final care plans, and help residents transition back into the community as outlined in their initial care plan.

To succeed in this position, you should have excellent analytical skills, effective written and verbal communication skills, and the ability to establish and maintain good relationships with all office, field, and administrative personnel.
