About the Role

Aveanna Healthcare is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in the comfort of patients' homes. As a key member of our team, you will be responsible for delivering and coordinating quality patient care in compliance with physician orders.

Key Responsibilities
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Participate in and update the nursing care plan to ensure effective patient outcomes.
  • Take appropriate nursing action based on assessment and achieve expected outcomes.
  • Recognize changes in patient needs and responses requiring intervention, implementing care to prevent or reduce risk.
  • Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
  • Provide care utilizing infection control measures that protect both staff and patients according to OSHA standards.
  • Educate patients and families regarding disease processes, self-care techniques, and prevention strategies to meet patient nursing needs.
  • Maintain knowledge of competencies related to the nursing profession by participating in educational programs, continued education units, internal learning management skills, and skill evaluations.
Requirements
  • 6 months of experience as a practicing nurse (new graduates welcome to apply for our excellent New Grad Program).
  • Nursing License (active and in good standing with the board).
  • CPR Certification (current, hands-on BLS).
Physical Requirements
  • Ability to speak, write, read, and understand English.
  • Ability to travel.
  • Ability to lift 50 pounds.
  • Ability to sufficiently reposition patients and move equipment without assistance.
  • Prolonged walking, standing, bending, kneeling, reaching, twisting.
  • Ability to sit and climb stairs.
  • Visual and hearing acuity.
  • Strong sense of smell and touch.
  • Ability to respond physically and mentally to emergency situations in the home or during transport.
Environment
  • Ability to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
