Ocean Gardens Care Center - Licensed Practical Nurse Role

POSITION OVERVIEW: The Licensed Practical Nurse (LPN) plays a crucial role in the Nursing Department by executing job responsibilities in alignment with established Federal, State, and Local regulations, as well as accepted standards of nursing practice. This position is guided by the directives of the Administration and/or the Director of Nursing.

  • REPORTING STRUCTURE:
  • Reports to the Registered Nurse
  • Reports to the Director of Nursing

KEY RESPONSIBILITIES:

  • Deliver direct patient care in accordance with individualized care plans, nursing protocols, and facility policies.
  • Monitor residents for any variations in their physical, social, or mental well-being and document pertinent information for further assessment.
  • Assist with the processes of admission, transfer, and discharge, providing essential education to residents.
  • Administer clinical services, including medication management, diabetic monitoring, nutritional assessments, and vital sign checks.
  • Engage in the formulation and evaluation of resident care plans.
  • Ensure precise medication delivery and adherence to treatment protocols.
  • Participate in care plan discussions while maintaining resident confidentiality as per established guidelines.
  • Review and update records related to resident care and medication administration.
  • Contribute to unit staff meetings and uphold a safe and hygienic environment within the nursing unit.
  • Identify the educational needs of nursing staff and facilitate necessary training.
  • Alert Administration to any safety concerns and undertake additional responsibilities as assigned.

  • Follow the facility's dress code and performance standards, and engage in mandatory training sessions.
  • Adhere to ethical principles and maintain professionalism in all interactions.

  • Exhibit sound decision-making, critical thinking, and adaptability in job tasks.

PROFESSIONAL COMPETENCIES:

  • Maintain required licensure/certification and demonstrate proficiency in the English language.
  • Employ critical thinking abilities, accept guidance, and work independently.
  • Show competence in job duties and uphold professional standards.
  • Physically capable of operating necessary equipment and executing essential job functions.
  • Utilize computer systems effectively to manage work outputs.

  • Carry out responsibilities throughout the facility while maintaining physical and emotional well-being.
  • Be prepared for potential emergencies and be willing to work beyond standard hours when required.
