We are seeking a highly skilled Physical Therapist Assistant to join our Skilled Home Care division at Interim HealthCare Home Health and Hospice. As a Physical Therapist Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our team.
Key Responsibilities- Participate in ongoing interdisciplinary assessments of patients to ensure comprehensive care.
- Act at the direction of the treating physical therapist and in accordance with the plan of care to provide patient-centered services.
- Collaborate with patients, representatives, and caregivers to develop and evaluate the plan of care, ensuring patient-centered goals and outcomes.
- Provide services ordered by physicians as indicated in the plan of care, promoting patient progress and well-being.
- Assist the treating physical therapist in documenting patient progress and response to treatment, ensuring accurate and timely documentation.
- Counsel and educate patients and their families, as well as other caregivers, to promote patient progress and meet healthcare-related needs.
- Effectively communicate with the interdisciplinary healthcare team to ensure coordination of patient care and planning for discharge.
- Communicate with physicians and other healthcare practitioners involved in the plan of care to ensure seamless patient care.
- Access and utilize personal health information necessary to fulfill duties and responsibilities, adhering to organizational and departmental guidelines.
- Current licensure as a physical therapist assistant in the state of Michigan.
- At least two years of experience as a physical therapist assistant in the last five years.
- Active CPR certification.
- Previous home health experience is a plus.
