We are a leading provider of residential and community-based services for youth and families.

Job Summary

The Clinical Transition Specialist will provide clinical services to youth and families transitioning from our residential program to the community. The ideal candidate will have experience working with youth and families in a mental health setting, possess excellent communication and interpersonal skills, and be able to work independently and as part of a multidisciplinary team.

Responsibilities
  • Serve as the primary clinical lead in providing therapeutic services to youth and families, traveling to their home and/or community setting as needed.
  • Develop and maintain a positive rapport and professional relationship with youth, families, and county representatives.
  • Maintain accurate and timely record keeping, including treatment plans, lethality and risk assessments, and other required documentation.
  • Meet established attendance criteria for meetings, events, and other obligations.
Requirements

To be successful in this role, you must possess:

  • A Master's Degree or Licensed Professional in a mental health field (LMSW, LMFT, LMHC, etc.).
  • Experience working in a mental health environment.
  • Knowledge of community resources and support systems.
  • Ability to work independently and collaboratively with a multidisciplinary team.
Salary and Benefits

This full-time position offers a competitive salary range of $66,000.00 - $70,000.00 per year, commensurate with experience. Additionally, we offer a comprehensive benefits package, including medical, dental, and vision insurance, retirement plan, and paid time off.
