Key Responsibilities
  • Deliver high-quality, safe patient care through a team-based model, incorporating virtual visits, group visits, and home visits.
  • Manage a population of adult patients with multiple chronic health conditions, utilizing strong clinical abilities and a patient-centric approach.
  • Collaborate with interdisciplinary teams, including digital mission partners, subspecialists, and other care entities, to ensure a seamless and effective care experience.
  • Maintain accurate and up-to-date patient medical records, including reports of all examinations, procedures, and other services performed.
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care and outcomes.
Requirements
  • Master's degree in advanced practice nursing.
  • IL Advanced Practice Nurse licensure and certification.
  • Experience with EPIC electronic health record system.
  • 1-2 years of experience as an Advanced Practice Nurse.
  • Excellent interpersonal and communication skills.
  • Solid computer skills, including proficiency with Microsoft software.
  • Strong analytical and problem-solving skills, with the ability to be detail-oriented.
