Per Diem Nurse Specialist

We are seeking a highly skilled and experienced Per Diem Nurse to join our team at the University of California, Irvine (UCI) Health. As a Per Diem Nurse, you will play a critical role in the Transfer Center, working closely with an interdisciplinary team to facilitate the transfer of patients to UCI Health.
Key Responsibilities- Conduct high-level assessments and reassessments of patients who desire to transfer to UCI Health, setting priorities and planning for successful transfer.
- Collaborate with the interdisciplinary team to coordinate high-quality, individualized, safe, and cost-effective patient care.
- Advocate for patient self-determination and choice, practicing cultural competence in evaluation, planning, and respect for patient and family diversity.
- Monitor and coordinate resource utilization throughout the health system, evaluating services needed or being provided by the sending facility.
- Perform utilization review functions using medical staff-approved decision support criteria.
- Minimum of 5 years recent nursing experience in Adult ICU or Adult ED.
- Strong knowledge of Joint Commission, CMS, CDPH, and CCS requirements.
- Excellent written and verbal English communication skills.
- Advanced computer literacy and proficiency in MS Office Products.
- Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload.
- Unit-specific nursing certification.
- Prior Transfer Center experience.
- Minimum of 2 years recent work experience in Case Management.
- Knowledge of University policies and procedures.
- Experience with Epic.
- RN.
- BLS.
- ACLS.
