We are seeking an experienced Nurse Practitioner or Physician Assistant to join our Complex Care Clinic team. In this role, you will be responsible for conducting pre-surgery evaluations and determining optimization needs based on algorithms and clinical pathways.
The Transitional Care aspect of this role focuses on preventing hospital readmissions by evaluating patients within the first week post-discharge, assessing their status, and treating/refer as needed. You will require strong evaluation and assessment skills, with limited diagnostic workups and prescriptions involved in this setting.
This role requires a Master's degree in Nurse Practitioner or Physician Assistant and 3+ years of experience working as an NP or PA. You must hold a Missouri registered nurse license and advanced practice nurse licensure or certified physician assistant.
