Duties and Responsibilities:
  • Assess patients' physical and emotional needs, developing targeted interventions to promote recovery and wellness.
  • Collaborate with interprofessional teams to coordinate care, share information, and address complex patient needs.
  • Implement individualized care plans, incorporating evidence-based practices and patient preferences.
  • Monitor patients' progress, making adjustments to care plans as needed to optimize outcomes.

Benefits and Compensation:
The estimated annual salary for this position is $70,000-$90,000, depending on experience, with opportunities for professional growth and development within our organization.
