Lead Social Work Manager, Northern California Home Health Agency

We are seeking a highly skilled and experienced Lead Social Work Manager to oversee the Medi-Cal Waiver Program at Home & Health Care Management in Chico, CA.
Salary: $32 per hour, negotiable based on experience and qualifications.
About the Role
- Supervise and mentor social work case managers to ensure high-quality service delivery and compliance with program requirements.
- Develop and implement tracking systems to monitor client engagement and program performance.
- Collaborate with cross-functional teams to achieve program goals and objectives.
Responsibilities
- Implement and manage the Medi-Cal Waiver Program (MCWP) contract, ensuring adherence to all caseload specifications and agency policies.
- Grow the MCWP case load to full capacity and meet caseload deadlines.
- Develop, monitor, and maintain a tracking system to schedule clients for regular contacts and report on program progress.
- Supervise case managers' documentation for quality, accuracy, and timeliness, providing training and guidance as needed.
- Review and approve Financial Assistance Requests as necessary.
- Schedule and assign caseloads to case managers, ensuring efficient workload management.
- Maintain open communication with interdisciplinary case conferences and attend when possible.
- Conduct home health visits to assess client needs and develop care plans.
Requirements
- Bachelor's degree in Social Work, Psychology, Counseling, Rehabilitation, or Sociology.
- At least one year of experience as a Program Manager or Supervisor.
- Excellent verbal, written, and critical thinking skills.
- Ability to inspire and motivate supervised staff, fostering a positive work atmosphere.
- Valid California Driver's License, eligible for liability insurance, and proof of current auto insurance.
Preferred Qualifications
- Master's degree in Social Work, Psychology, Counseling, Rehabilitation, or Sociology.
- Three years of experience as a Program Manager or Supervisor.
- Prior knowledge of HIV/AIDS services and working knowledge of benefits programs specific to this population.
- Fluency in Spanish and excellent interpersonal, problem-solving, and written/oral communication skills.
