As a Physical Therapist with JAG Physical Therapy, you will have the opportunity to work with patients in their homes, providing personalized care and rehabilitation services. • Collect and record patient care data, including patient history, diagnoses, and treatment plans.
• Provide education and training to patients and their support systems on therapeutic activities, rehabilitation equipment, and community reintegration.
• Develop and implement individualized treatment plans, including exercise prescription and modalities.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Document patient progress and outcomes in our electronic medical record system.

• Bachelor's or Master's degree in Physical Therapy required.
• Licensed Physical Therapist in the state practicing in.
• Completed state-regulated CEU requirements in appropriate state (NY, NJ).
• Valid CPR license.
• Experience with EMR software preferred.
• Excellent communication and clinical decision-making skills.
